A patient with typical ECG features of arrhythmogenic right ventricular cardiomyopathy

Abstract

|
Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a type of progressive cardiomyopathy characterized by fibrofatty infiltration of the myocardium, especially the right ventricle. This pathologic process may induce typical ECG features of ARVC such as T-wave inversion and localized QRS widening in the right precordial leads and epsilon wave. We report the case of a middle-aged woman who presented with sustained LBBB-type ventricular tachycardia with inferior axis. Routine ECG after DC cardioversion showed T-wave inversion in V1~V4 and epsilon wave. RV enlargement and apical aneurysms of both ventricles were also observed on 2-D echocardiography. Diagnosis of ARVC was possible with only routine ECG and 2-D echocardiography.
